To improve the health and well-being of our children

The first step of the Highmark Healthy High 5 grant application process for 501(c)3 nonprofit organizations is to complete the eligibility screening. Eligible nonprofits must then submit to the Highmark Foundation a two- to three-page letter of inquiry. A letter of inquiry must be submitted on organizational or program letterhead, and must be signed by the administrative head of the applicant organization.

A letter of inquiry must include:

  • Organizational overview (mission and contact information)
  • Project name/title and area(s) of health promotion to be addressed
  • Critical needs statement: statistics to support the project need, how the project is filling a community gap, whether other community agencies are involved in this program
  • Project overview and key activities
  • Project goal(s), objective(s) and anticipated outcome(s)
  • Measurement methodology (include a description of the measurement tool)
  • Target population addressed (the number of children who will be served by the program, parental involvement in the program)
  • Specific amount being requested and an itemized program budget

Upon receipt and review of a letter of inquiry, the Highmark Foundation will notify an applicant about the status of a request. If a request meets all criteria, Highmark Foundation will invite the applicant to submit a formal proposal. Learn more about the Highmark Foundation.
