Letter of inquiry
All requests from eligible 501 (c) 3 nonprofit organizations for Highmark Healthy High 5 funding must first submit a two- to three-page letter of inquiry to the Highmark Foundation.
Please complete our eligibility screening to determine if you should submit a letter of inquiry.
The letter of inquiry must be submitted on your organization or program's letterhead and must be signed by the head of the applicant organization. You can submit it as an attached Word document in an e-mail or by U.S. mail (see addresses below).
The letter of inquiry must include:
- Organizational overview (mission and contact information)
- Project name/title and area(s) of health promotion to be addressed
- Critical needs statement: statistics to support the need for your project, how your project is filling a gap in your community, whether other agencies are doing this program in your community
- Project overview and key activities
- Project goal(s), objective(s) and anticipated outcome(s)
- Measurement methodology (attach copy of measurement tool)
- Target population addressed (the number of children who will be impacted by the program, parental involvement in the program)
- Itemized program budget and specific amount requested
Upon receipt and review of the letter of inquiry, the Highmark Foundation will notify the applicant of the status of the request. If it is determined that the request meets the criteria, a formal proposal will be requested.
